Our Personal Body Silhouette Questionnaire will help  to determine the correct styles and cuts of clothing that suit your body-type.   It  is the perfect solution, especially  if you  don't have the time sit down for a personal consultation.  The forms are simple to fill out, and  they are  custom designed  to suit  your needs, at your convenience. 

After you place your order, we will e-mail you your Personal Body Silhouette Questionnaire, (you will need Adobe Acrobat 6.0 or higher to use the form).

Please answer all of the questions as accurately as possible, and e-mail it back to us.

Please provide the following items, so that we may accurately determine your body-type. 

1.  One colour photo full shot of your body, standing straight with your arms at your sides and feet together (preferably dressed in a black bodysuit against a white background).

2.  One colour photo of a close-up your face in natural daylight (with a relaxed facial expression e.g. no smiling etc.) without make-up, and all of your hair pulled back off of your face.

3.  One colour photo of a close-up your face in natural daylight (with a relaxed facial expression e.g. no smiling etc.) with make-up, and your hair styled as it is usually worn.

4.  Three or four colour photos of you dressed in your favourite outfits (e.g. casual, business & dress).
